Abstract

Objective: The aim of this study was to explore the association between phosphodiesterase-5 (PDE5) inhibitors and men with erectile dysfunction (ED), and to evaluate the risk of side effects.

Design: A retrospective cohort study.

Setting: The Netherlands.

Participants: Men with ED, who were prescribed sildenafil citrate for ED and sildenafil for pulmonary hypertension.

Interventions: A prospective cohort study, followed by two-fold stratification: first, the use of sildenafil citrate as treatment for ED was compared to sildenafil citrate as treatment for pulmonary hypertension; second, the use of sildenafil citrate as treatment for ED was compared to sildenafil citrate as treatment for pulmonary hypertension; third, the use of sildenafil citrate as treatment for ED was compared to sildenafil citrate as treatment for pulmonary hypertension.

Results: The prevalence of ED and its impact on men's sexual satisfaction was higher in the sildenafil group (17.3%) than in the sildenafil citrate group (11.1%). Moreover, the use of sildenafil as treatment for ED was associated with more negative erectile symptoms (7.7 vs. 5.6%, p = 0.001) compared to sildenafil citrate as treatment for pulmonary hypertension. The risk of adverse events was similar in both groups. There was a significant association between sildenafil citrate as treatment for ED and men with ED with an adjusted hazard ratio (HR) of 0.99 (95% confidence interval [CI] 0.87-1.16). The use of sildenafil citrate as treatment for ED was associated with less sexual satisfaction than sildenafil citrate as treatment for pulmonary hypertension.

Conclusion: There was no significant association between the use of sildenafil citrate as treatment for ED and men with ED.
